Root Cause Analysis

  • Purpose
    • Determine the implicity form of a problem
  • Description
    • Structured examination of aspects of a situation to establish the root causes and effects resulting from the problem
    • Key element to ensure challenge the thought of the business and processes
  • Elements
    1. Diagram Fishbone / Ishikawa / Cause and Effect
      • Capture the question in a box at the top of the diagram
      • Draw lines on the categories of potential causes
      • Draw minor lines on the deeper causes
      • Use brainstorming to search for categories and causes
      • Analyze the results (future detail)
      • Use brainstorming to find solutions
    2. Five whys
      • Question and answer process to explore the nature and cause of a problem, through repeated questions
      • Write the problem and ask (1x + 4x):
        • “Why do you think the problem occurs?” + “Why?”
      • Use singly or as part of the diagram of Ishikawa
  • Usage Considerations
    1. Advantages
      • Provides a structured method to identify the root causes of the identified and understood problems
    2. Disadvantages
      • It works better when someone trained facilitates the experts team, the facilitator must remain objective
